L\u2019esame rappresenta lo strumento pi\u00f9 importante ai fini della valutazione della fertilit\u00e0 maschile. Si realizza a livello macroscopico e microscopico, analizzando parametri quali pH, volume, fluidificazione, viscosit\u00e0, conta degli spermatozoi, motilit\u00e0, vitalit\u00e0 e morfologia. Se il campione presenta meno di 2 ml, ci troviamo di fronte ad un caso di ipospermia. Un volume inferiore potrebbe indicare un\u2019ostruzione provocata da un\u2019infezione genitale, alterazione congenita dei dotti deferenti o eiaculazione retrograda.<\/p><p><strong>Fluidificazione:\u00a0<\/strong>appena emesso, il campione coagula, per eseguire l\u2019analisi deve liquefarsi. La liquefazione avviene dopo 20-30 minuti. La mancata liquefazione pu\u00f2 essere correlata ad alcune disfunzioni a livello prostatico.<\/p><p><strong>Viscosit\u00e0:\u00a0<\/strong>lo sperma, dopo la liquefazione, deve essere leggermente pi\u00f9 viscoso dell\u2019acqua. L\u2019eccesso di viscosit\u00e0 di un campione pu\u00f2 essere causato da disfunzione prostatica, eiaculazione frequente e\/o uno stato psicologico del paziente. Tale aumento di viscosit\u00e0 non costituisce una causa diretta di sterilit\u00e0, ma pu\u00f2 avere ripercussioni su successive valutazioni dello spermiogramma, quali concentrazione e motilit\u00e0 spermatica.<\/p><p><strong>Conta spermatica:\u00a0<\/strong>i valori normali devono essere superiori a 20 milioni\/ml. Se il valore \u00e8 inferiore il campione riceve le seguenti classificazioni in funzione del numero di spermatozoi:<\/p><ul><li>Oligozoospermia moderata: tra 10 e 20 milioni\/ml<\/li><li>Oligozoospermia severa: tra 0,1 e 10 milioni\/ml<\/li><li>Criptozoospermia: meno di 0,1 milioni\/ml<\/li><li>Azoospermia: assenza di spermatozoi nell\u2019eiaculato<\/li><\/ul><p><strong>Motilit\u00e0:\u00a0<\/strong>la motilit\u00e0 si basa sull\u2019osservazione del campione mediante microscopio. Si definiscono diverse tipologie:<\/p><ul><li>Classe a) Eccellente. Spostamenti rapidi con moto rettilineo. (Classificata anche come \u201c+++\u201d)<\/li><li>Classe b) Moderata. Velocit\u00e0 progressiva moderata con moto rettilineo. (Classificata anche come \u201c++\u201d)<\/li><li>Classe c) Velocit\u00e0 irregolare con moto poco rettilineo o non rettilineo. (Classificata anche come \u201c++\u201d) o movimento dello spermatozoo senza appena spostamenti. (Classificata anche \u201c+\u201d)<\/li><li>Classe d) Assenza di motilit\u00e0<\/li><\/ul><p><strong>Vitalit\u00e0:\u00a0<\/strong>la vitalit\u00e0 ci consente di conoscere il numero di spermatozoi vivi, bench\u00e9 non presentino movimento. Per la rilevazione si impiega una colorazione in grado di evidenziare gli spermatozoi morti. Quando il valore \u00e8 inferiore al 60%, il campione viene definito \u201cnecrozoospermico\u201d.<\/p><p><strong>Morfologia:\u00a0<\/strong>\u00e8 lo studio della forma dello spermatozoo. Lo studio si esegue applicando i criteri stretti di Tygerberg, in base ai quali un campione viene considerato morfologicamente normale se il numero di spermatozoi con forma tipica \u00e8 uguale o superiore al 4%.<\/p><p>\u00a0<\/p><p>\u00a0<\/p><p>\u00a0<\/p><p>\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-85565d6 elementor-widget elementor-widget-heading\" data-id=\"85565d6\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Capacitazione Spermatica\n\n<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-81428fb elementor-widget elementor-widget-text-editor\" data-id=\"81428fb\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Per capacitazione spermatica si intende il processo che consente di separare il liquido seminale dagli spermatozoi. Avviene \u201cin vivo\u201d. Nel laboratorio, questa tecnica ci consente di separare il liquido seminale e, inoltre, gli spermatozoi di buona qualit\u00e0 da quelli di qualit\u00e0 insufficiente. \u00c8, in sostanza, un processo di selezione. Permette di rilevare il numero e la motilit\u00e0 degli spermatozoi recuperati, fornendo orientazioni utili, qualora siano necessarie, sulla tecnica di Riproduzione Assistita pi\u00f9 idonea.<\/p><p>\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ba4fd97 elementor-widget elementor-widget-text-editor\" data-id=\"ba4fd97\"
