In tegenstelling tot traditionele IVF, waarbij de eicel in het laboratorium met sperma wordt gemengd en bevruchting spontaan plaatsvindt, wordt bij ICSI het sperma rechtstreeks in de eicel gebracht via een zeer gecontroleerd en nauwkeurig proces, de intracytoplasmatische en voorbereid in het laboratorium. Vanaf dat punt wijkt de behandeling aanzienlijk af. Zodra de follikels de juiste grootte hebben bereikt, vindt een eicelpunctie plaats, een minimaal invasieve procedure waarbij eicellen worden teruggehaald. Deze eicellen worden verzameld voor verwerking in het laboratorium.<\/span><\/strong><\/p><p><strong>2.<b>Denudatie van de o\u00f6cyten<\/b><\/strong>\u00a0<\/p><p><span style=\"font-weight: 400;\">De opgevangen eicellen zijn omgeven door een laag cellen die verwijderd moet worden om de rijpheid van de o\u00f6cyten te beoordelen. Dit proces heet \u201cdenudatie\u201d. Alleen o\u00f6cyten die het Metafase II-stadium hebben bereikt (ongeveer 80%) zijn geschikt voor spermamicro-injectie.<\/span><\/p><p><strong>3.<b>Micro-injectie van sperma (ICSI)<\/b><\/strong><\/p><p><span style=\"font-weight: 400;\">De ICSI-procedure wordt uitgevoerd met behulp van een gespecialiseerde micromanipulator met een omgekeerde microscoop die een vergroting van 400x biedt. Met een micromanipulatiearm wordt de eicel vastgehouden terwijl met een micro-injectorarm een geselecteerde zaadcel rechtstreeks in het cytoplasma van de eicel wordt ingebracht. Dit verzekert bevruchting, zelfs bij een lage beweeglijkheid of kwaliteit van het sperma, en vermindert de kans op mislukte bevruchting.<\/span><\/p><p><strong>4.<b>Embryokweek in gecontroleerde incubators<\/b><\/strong>\u00a0<\/p><p><span style=\"font-weight: 400;\">Na de micro-injectie worden de o\u00f6cyten in incubators geplaatst die de fysiologische omstandigheden van het menselijk lichaam nabootsen: 37\u00b0C, 6% CO2 en hoge luchtvochtigheid. Ongeveer 16-18 uur na injectie wordt gecontroleerd of de bevruchting heeft plaatsgevonden.<\/span><\/p><p><strong>5.<b>Ontwikkeling van embryo\u2019s en terugplaatsing in de baarmoeder<\/b><\/strong>\u00a0<\/p><p><span style=\"font-weight: 400;\">De bevruchte eicellen (embryo\u2019s) worden in het laboratorium verder gekweekt tot dag 5 of 6, het blastocyststadium. Deze verlengde kweek maakt een betere selectie van embryo\u2019s mogelijk, wat de kans op succesvolle implantatie vergroot. Vervolgens worden \u00e9\u00e9n of meerdere embryo\u2019s teruggeplaatst in de baarmoeder.<\/span><\/p><p><strong>6.<b>Cryopreservatie van overtollige embryo\u2019s<\/b><\/strong>\u00a0<\/p><p><span style=\"font-weight: 400;\">Als er meer embryo\u2019s van goede kwaliteit zijn dan nodig voor de eerste terugplaatsing, worden deze ingevroren (gecryopreserveerd) voor toekomstig gebruik. Dit voorkomt dat de pati\u00ebnt opnieuw een stimulatiecyclus hoeft te ondergaan bij een tweede
